How we track progress

We agree small, measurable goals in individual support plans. Key workers use session records, progress reports and Outcome Stars to review achievements, understand barriers and agree the next steps with the young person.

Plans are reviewed monthly with the young person and every three months with their social worker, and updated when needs or risks change.

Planning & review

Support that responds
to changing needs.

A designated key worker translates agreed goals into practical support sessions. The young person participates in setting priorities and reviewing progress.

Building independence safely

Support levels are based on assessed need. We explain the agreed staffing and support arrangements during referral and placement planning.

We take a trauma-informed, strengths-based approach: recognising experiences, understanding what helps and avoiding unnecessary pressure. We use positive communication and de-escalation. Our Statement of Purpose says that staff do not use physical restraint.

Healthcare and medication

We help young people access healthcare and develop safe routines for managing their own medication. We do not administer medication. Any medication-related risks are considered with the placing authority and relevant health professionals.

Specialist treatment is provided by the appropriate professionals. A referral assessment considers whether supported accommodation can meet the young person’s needs safely.
